deadly wasps and deadly labour



my last oncall in paediatric posting surely interesting. memorable.

i was a passive HO oncall that day aka covering the labour ward calls, OT and taking care of nursery well. like my other oncalls, i was considered "cold". no admission, no prematures babies with problems, no intubation. i can sleep after 12mn undisturb. thank you. and like those calls, my last call was quiet. the babies in the nursery were behaving well. no desaturation, no funny and weird blood gases. no funny and weird orders from my funny and upbeat MO. she was not in the ward as usual which i found it kind of weird because when i oncall with her previously, she will stay in the ward until at least 12mn, make sure all babies have a good night sleep and behave well, or find any femoral veins or arteries to poke, or making herself busy. with her absence, is actually a good news for me...yokattaaaa...i can sleep early tonight!!!that was what i thought. suddenly my question was answered by a phone call. "Wani, datang to paed Iso, EMERGENCY"

i saw my specialist, my MO and my colleague resuscitating a child. "Wani, turun bawah, tolong Dr N resus patient dekat bawah"...EHHHHHH???>..2 resuscitaion at 1 time???what the hell is going on??

it turned out to be a family (a mother with 3 children and her brother) were stung by hundreds of wasps. all the children and the 10year old boy(the uncle) were admitted to our ward. from what i've heard initially,they came in all comfortable. went in walking, no shortness of breath. FINE. however after 2hours, 1 by 1 of them collapse. meaning, suddenly became breathless, drowsy. Anaphylactic shock. within 2hours of admission. 2 of the siblings died. the elder sister currently now in the ICU, not doing well. last time i heard, she requires high ventilator settings and BP was unstable. even the consultant paed anaesthesiologist couldn't make herself at ease. trying her best to save the child. the mother also in ICU, stable. only the uncle discharged home well. the father was still ok. not insane, not losing hope yet. cheering her last child to fight for life. praying hard not to loose the daughter and the wife. if you are the father, are you going to lose yourself by now?..i may even thinking of suicide. losing the whole family in a night. it was dramatic. too dramatic.



other than that, when i passed Labour ward after resuscitated a baby, i noticed this.





"I'm in labour!!!!!"

chigau.... "I'm having abortion!!!!"

scary eh..even though estimated weight was 488gm from scan, hence it considered miscarriage, hence no resuscitation is needed. but, what if after the delivery, the baby still alive and when they weigh the baby it turns out to be 500gm?...intubate a 500grammer? AHo ka? Baka.

i told the O+G HO, "make sure the baby is less than 500gm!!"

i don't know the progression though. maybe she already delivered.


counting days. can't wait for this posting to end. the kids are so adorable. but the posting itself is a torture.

a little update:-
1) michael Jackson went home safely with the care of the ever "brilliant" mother..pray hard.
2) Errica is going to KL to get her bone marrow transplant..yeah for Errica!!!

we, the ED HOs, yg attend those kids had been contemplating for days, of what we cud hav done to avoid death initially..

sigh...
